1. Go to evenium.com 
2. Click My Events
3. Click the name of the event under the I organize section
4. Go to the Configuration tab located on the purple tab
5. Click Registration Form

There are two sections on the registration form: Personal Information and Your Questions. In Personal Information you can choose to add questions from an existing list by clicking Add a personal field. If you want to create your own custom questions, click on Add a question under the Your Questions section.
